"Killer" is just a subtype that usually means the icebreaker breaks Sentry subroutines. I believe "Decoder" is the equivalent for code gates and "Fracter" is the equivalent for walls. So far, it doesn't have any gameplay significance, as the abilities on the icebreaker are what really determines what types of ice it can break. In the original Netrunner, I believe there were some cards with recurring credits that could only be spent on Killer icebreakers (e.g. Corolla Speed Chip), so Android:Netrunner might see something like that in the future.

Assets like Pad Campaign do go in remote servers. You do not have to rez an asset when you install it; the most tactical time to rez a Pad Campaign (or Adonis Campaign, the Haas-Bioroid version of Pad Campaign) is probably at the end of the Runner's turn, since that guarantees you'll get at least one start-of-turn payout from it after paying to rez it.
